We're recruiting for a Personal Advisors to support young people aged 16-25, helping them develop independence and successfully transition into adulthood.
Key Responsibilitiesand#10004;and#65039; Lead and review Pathway Plans
and#10004;and#65039; Support young people with accommodation, finances, benefits and healthcare
and#10004;and#65039; Provide consistent guidance and emotional support
and#10004;and#65039; Coordinate with social workers, housing, education and health services
and#10004;and#65039; Advocate for young people and their best interests
and#10004;and#65039; Maintain regular contact and monitor wellbeing
and#10004;and#65039; Keep accurate and up-to-date case records
